2. Hand-print, legibly, your full name, address (including city, state and zip code), phone number and e-mail address (if you have one) in the space provided on this original form (No P.O. Boxes permitted), AND
3. Mail the form to the address noted below* along with the following proofs of purchase:
• The UPC bar code you have cut from the original product package(s) (graphic of UPC) AND
